My uneducated guess is fake. Never seen anything like that before from IWC, neither case nor dial.
Looks very strange to me but I'm not a pocket watch expert.
Is that a pocket watch? What pocket would it fit in?
Can't imagine, IWC would have ever done such a strange (and ugly) piece.

Looks like a pirate's deck watch. I believe Blackbeard had severeal of these comissioned for his most trusted lieutenants.
